I started spinning, hmmm, two years ago I guess. Spinning with fire for one year.

Was welcomed into a troupe quite quickly www.firenmotion.com (website is getting reworked with spiffy animations.. very excited)

Being in the troupe is great, mostly because it holds my best friends, my med school buddies and my partner. hug



I got bored of only doing poi pretty fast, and decided to make my own firechucks. With no firetoy making experience, I think it went pretty well!!



I went to the Perth Annual Fire-Camp, SpunOut, and will hopefully also be at 2UberOz.



Between a choice of becoming a fulltime fire performer and a trauma surgeon, I know which one would be more enjoyable, but my heart strings are tugged with a sence of responsibility to use the skills that I have... So don't be confused when a series of firespinners start appearing from poverty stricken areas of Africa in the future... biggrin

I'm guessing marmite is available in Aus.. I've never tried it.
And I don't like nuts... so not Nutella either.

I can handle vegemite, as long as it's not smothered.

This is probably.. the fourth time I'd spun with fire... 6 months ago. I don't have a nunchuck/martial arts base either... I just picked them up once and went "oooh, these are fun"!

How about you?? Are their other firechuckers where you are. I'm sad and lonely in perth... though I am teaching the other members of my troupe and we're trying doubles now etc.. hopefully we'll be able to perform with them soon!!

How did you make yours?

I have since re made mine because I used metal, kevlar wick and thread and nylon rope (which was rubbed through) so I have actual chuck heads on them now.

Am interested in joining the firechucking community here!!

Ummm...isn't acute sinusitis just a bad cold?...and if it is, then no amount of antibiotics in the world is gonna help...but if you're referring to a sinus infection, then I think ampicillin is pretty much a standard...altho, I could be wrong... shrug

(This exercise is called "pimping" for the non-medical folks out there. It's the process by which you grill a medical student to assess their knowledge.)

I pretty much grew up in the lab of the local hospital...my dad was the supervisor...plus I've got two kids...you learn more than you realise...

I know that erythromycin is a pretty common antibiotic for a wide range of infections...how bout that one?

You could use a macrolide, like erythromycin, although the patient would probably prefer a short course of azithromycin or clarithromycin. The problem is that in our hospital's population, fully 30% of organisms isolated from pateints with head and neck infections are resistant to the macrolide class of antibiotic, due to over-use of the famed "Z-pack."



However, the recommended antibiotic for sinusitis that fails amoxicillin or for sinusitis in high-risk patients (immunocompromised, exposed to children, works in a healthcare setting, dorm/prison/shelter resident, etc.) is amoxicillin/clavulanic acid or AUGMENTIN. It should be given in a high dose. The high-dose amoxicillin is actually enough to overwhelm penicillin-resistant S. pneumoniae while the clavulanic acid can overcome penicillin resistance in the beta-lactamase producing strains of Moraxella and H. influenzae. It also can fight a broad range of gram-positive and gram-negative organisms AND anaerobes, which can get involved in chronic sinusitis. For the patient who has been admitted to the hospital, UNASYN (ampicillin/sulbactam) is the preffered antibiotic for IV therapy. Note that this does not apply if there is clinical concern for secondary meningitis/cerebral empyema (but you can look up what you should do in such a case).



For the true penicillin-allergic patient, you can use a 2nd or 3rd generation cephalosporin (cefuroxime [CEFTIN], cefpodoxime [SUPRAX/VANTIN], cefdinir [OMNICEF] are usually preferred) and add either clindamycin or metronidazole for anaerobic coverage, although the latter is somewhat lacking in gram-positive coverage.



As you probably know, 10% of patients who are allergic to penicillins are cross-allergic to cephalosporins. If you're too skittish to use a cephalosporin, or if the patient's allergic reaction is frank anaphylaxis (it's so rude when they do that!) then a broad-spectrum fluoroquinolone, like ofloxacin, or its levo-enantiomer, levofloxacin, mixed with either clindamycin or metronidazole for anaerobic coverage. But keep in mind that fluoroquinolones do have the theoretical risk of increased rate of tendon rupture, and more importantly, that fluoroquinolone resistance is common, especially among gram-positives, so clindamycin may be a better choice than metronidazole. In children, where fluoroquinolone use is still highly controversial, trimethoprim/sulfamethoxazole (BACTRIM/SEPTRA) is appropriate, although many populations are now gathering high rates of resistance to this agent and it will also require additional coverage for anaerobes.



And this is the useless crap I like to stuff my head full of. A simple "Augmentin" would have sufficed, no?

Sorry, I have been studying or enjoying my new found freedom.

And since exams are over, the only things I remember are useful acronyms that my friends made for me.

Staphs - FCC
Streps - PCC...
Anaerobes - MC

Now to just remember what the acronyms stand for...
