Questionnaire about a hospital logo

1.do you want it to be modern? Nope
(yes, but not too flashy: it smacks of NHS trust marketing)

2.how simple do you want it to be?
(simple as possible)

3.what sort of colour scheme do you want?
Primary colours, with the trust/hospital name appearing in a more formal monochrome

4.are there any shapes or designs you want to incorporate?
free form shapes, contained by a more formal structure (eg. rectangular border)

5. What sort of style would you like it to be?
I would take the style of a childs drawing, eg. segment of a rainbow, with chalk texture. but contained within a more structured border with trust name appearing outside it

6.which of these styles do you prefer? *new style one all neon* *older traditional style one*
(simple, modern, clean looking, with a tradtional font for hospital trust name)

7.should it contain text?
(As little as possible)

questinare about a payslip

note: I assume you're asking a company representative what they want, rather than recipient of it.

Do you want it to update automatically?
Yes (it's on a computer then is it?)

Do you want to display you’re logo at the top?
(yes, but not too obtrusively.. the recipient will know who they are working for anyway)

Do you want it to mail merge?
(Yes, I think all modern payslip systems merge automatically with database profiles now anyway.. don't they?)

Which layout do you prefer? *excel style one* *other style with bold clear sections for each part*
(Not excel.. to many cells staring back at you
Clarity is most important, so nothing more than subheading and an amount... eg.
Gross : £££
Deductions : £££
Net £££

Do you want it to have a header for:
National insurance? yes
Pension fund? yes
Tax deducted? yes
Income tax? yes
Gross and net pay? yes

For clarity: All deductions as subsections in one main section "deductions".. all tally up
