Posted:So I have this nasty cold. I'm shivering my lil' buns off. I don't have a home thermometer (I know, I know, baaaad med student!), but I can tell I'm febrile.

I'm resisting the urge to take Tylenol because there is some evidence that reducing fever with Tylenol prolongs overall disease duration (which makes sense...fever is there for a reason).

But it sucks, and I can't breathe and I feel like crap and I can't even ask anyone to hug me because I don't want them to catch it! ubbcrying Waaaaaah!

Posted:It was sort of amusing. I went into the drug store at the hospital today to buy some Tylenol. the lady behind the counter (a volunteer, not a pharmacist) was all "Tylenol Cold and Flu is better for a cold, honey." I was dressed casually (cargo pants, ripped T-shirt, polar fleece pull-over) so she didn't realize that I usually wear a White Coat.

At this point, I held up my entire armentarium of drugs (Afrin, Vitamin C, Zinc lozenges, cough drops, etc.) and said, "I think I'm fine, thanks."

Anyways, my guess is that all my potions and pills will make me get better within 5-7 days. And if I do nothing, I'll get better in a bit under a week. rolleyes

When I was in college, I didn't get sick for five years. Five years! Not even a case of the sniffoos. Then I got to med school and I started getting sick once or twice a year (almost certainly from the stress).

But this year, combining the stress with exposure to all sorts of viruses from the little kids I've been working with (I know exactly which patient I caught this bug from...and why the poor little thing looked so miserable), I've gotten sick three times already. Twice were mild nose colds, but this one knocked me flat over and ran me over a few times for good measure.

Posted:the peace corps has an interesting rule of thumb - let a low grade fever stew on its own, but take fever reducers for a high grade fever (102 F or more). I think that is where you get the chills and sweats and stuff anyway (is that what febrile means?)

Posted:I've actually known a few doctors to get really sick. I saw a doctor in the ER who was having a heart attack. (Would have been nice if he'd told me up front he was a doctor instead of having me patronize him for the first 15 minutes by explaining what an EKG is...boy was I embarassed when he said "Oh, I'm an internist.") A good friend of the family who is a general surgeon had to have a right hemicolectomy for colon cancer. Another friend of mine blew a brain aneurism (he's a general surgeon). And a friend of mine who is a hematologist is scheduled for knee surgery on Dec. 22.

So it happens. The nice thing is that when we show up to a doctor's office, we get amazing care. It also helps that because we all speak the same language, what would normally be a 15 minute discussion with most patients takes about 2 minutes with a doctor, since you can simply tell him the diagnosis (which he probably already knows) without having to explain it. Since entering medical school, I've never had to wait more than 15 minutes to be seen and I've never been charged a cent for any office visit. Professional courtesy is a good thing. biggrin

Van, we normally put 102 as our cut-off for starting to get concerned, although in my opinion, this is unnecessarily conservative. I don't worry about brain damage until about 41C (105F).

An idea we try to pound into parents' skulls is that fever is not necessarily a bad thing and that it is possibly to be treated, but not controlled. Parents are terrified of fever and will call the doctor if the kid is 99F or above, when really, such variations in temperature are routine and need not signify a serious illness.

There are some reports from human data that giving antipyretic (anti-fever) drugs like tylenol increases virus shedding and symptom duration in the common cold and delays crusting of blisters in chickenpox.

Having said that, Tylenol is a great therapeutic drug if the fever becomes bothersome (for example, I had a day full of required lectures yesterday and couldn't be bothered to be febrile).

Posted:Yeah, that's what I've heard about the British medical system is that the waiting time for problems is horrible.

However, like abroad, scheduling a specialist appointment here can be awful. It's just that most doctors make an effort to get into see students or other doctors, even if it means some schedule juggling.

I had tick bite fever before and besides the worst headache imaginable, the fever was pretty cool. It was deadly sleeping 23 hours a day and drifting in and out of consiousness and the dreaming and stuff ubblove ubblove

Posted:Well, I'm pretty much over it except for this really annoying drainage into my lungs, especially when I am lying down at night. Last night, my boyfriend and I were lying in bed together and every hour I kept waking up and hacking away and he kept rolling over and going "Are you OK?" I was like "yeah...no worries...*HACK! COUGH! WHEEZE! GASP!* I'm fine. *COUGH! COUGH! HACK! ARRHEM-HEM!!!*" Really sexy. ubblol Eventually, I just got out of bed and went to the bathroom to cough.

This morning, I had one of my roommates listen to my lungs with his stethoscope and he heard some wheezes and junky breath sounds in the right lower lobe, which is where drainage from the nose and mouth tends to settle, but no crackles or anything, which would suggest a pneumonia. And besides, I'm not running a fever. But it's kind of annoying.

Furthermore, yesterday I saw a patient with a really bad viral gastroenteritis, which isn't a serious disease, but it will make you really sick for a day or two (which I can't afford right now). I washed my hands like six times after seeing him and washed my face and used disenfectant hand gel, but I bet I'll get it. rolleyes
